The agency has run its home and community-based programs on ServTracker since 2009, expanding from a single congregate meals module into home-delivered meals, home care, transportation, and dozens of other services. Under the combined efforts of the Board, the IT committee, the agency’s CEO Chuck Gehring, and VP of Operations Molly Haroz, ServTracker has become the operational backbone that lets LifeCare Alliance report on demand, document every client interaction, add new counties and menus without breaking workflows, and support the whole person across programs.

How LifeCare Alliance Uses ServTracker to Grow Programs, Prove Impact, and Serve the Whole Person  

LifeCare Alliance is a statewide multi-service social service agency and one of the largest Meals-on-Wheels providers in the country. Home-delivered meals is its largest program, alongside congregate dining, client transportation, home care, and a wide range of other services tracked in ServTracker’s other services module across multiple counties. 

LifeCare Alliance began using ServTracker in 2009, starting with the congregate meals module, moved home-delivered meals onto the platform in 2012, and has folded nearly every other home and community-based program in since. Today, everything outside the clinical EHR lives in ServTracker. 

The Challenge

Homegrown Databases That Reset Every Day

Before ServTracker, LifeCare Alliance ran its programs on homegrown Microsoft Access databases with two structural problems. Operational staff could not pull their own reports; every request went through IT and often came back missing the fields the team actually needed. 

The bigger problem was that the database reset every day. “Literally, the Access database reset every day. Everything from the day before was just deleted once we hit the refresh button, which was necessary to create that day’s new kitchen order,” Haroz says. “It’s like Groundhog Day.” The team could see service unit totals but had no visibility into what went into them, no way to confirm accuracy, and no history to work from when a client’s situation changed over time. As programs grew more complex, the gap between what leadership needed and what the system could deliver kept widening.

Case Notes That Give Staff Full Context

Detailed, timestamped case notes give staff a shared view of every client interaction. When a client calls, staff can see whether the person calls in often, whether they have raised concerns before, and what has been discussed, helping the team deliver more appropriate and responsive customer service.

A Query Builder That Answers Any Question

Evaluation and reporting run on ServTracker's query builder. Once staff wrap their heads around it, they can pull any dataset the organization needs, which has been especially valuable for grant writing where the team can tell a compelling, specific story about the populations LifeCare Alliance serves.

Statewide Footprint

LifeCare Alliance has recently started using ServTracker's mapping features on the home-delivered meals side to support route planning and coverage analysis across its five-county footprint.

Data Transfers That Unlock New Funding

ServTracker's ability to move unit data into external systems has been foundational to LifeCare Alliance's growth into new funding relationships, including managed care organizations that pay for home and community-based services and federal reimbursement pathways. That kind of data transfer would not have been possible on the homegrown Access databases, and without it the organization could not have expanded into many of the funding sources it relies on today.

Since implementation, LifeCare Alliance has expanded into additional counties, added new menu types and diet options, and taken on more complex funding relationships. The platform's flexibility has helped the agency keep pace as more for-profit meal providers have entered the space, giving the team a way to test new offerings without disrupting existing operations.

With reporting and documentation running smoothly, leaders spend less time chasing information and more time thinking strategically. Deep familiarity with the platform has also made it easier to grow other programs into fuller use of ServTracker, since skills transfer readily between modules.

Documentation That Protects the Mission

Timestamped case notes and referral records have become a critical layer of accountability. When questions were raised about a vulnerable client's care, LifeCare Alliance was able to pull a complete, chronological record of case notes and referrals, as well as the follow-up attempts across weeks. The same documentation supports staff training and coaching when data entry practices need reinforcement.

When Area Agencies on Aging conduct monitoring reviews, the team can pull a full client picture, including profile, services, and notes, into a single bundle. That makes site visits and audits faster and gives funders confidence in the underlying data.

Haroz points to platform adoption itself as one of the wins she is most proud of. "Getting everybody in it, so everybody's in it, and really winning over some people who said it was the worst thing that ever happened," she says. "We don't have 100 different spreadsheets that people are keeping track of anymore."

ServTracker Coming in Clutch

Emergency Data Migration Saved Hundreds of Hours and Thousands of Dollars

In July of 2026, LifeCare Alliance unexpectedly had to take over the Older Americans Act home delivered and congregate meals for Darke, Greene, Miami, and Montgomery counties. The previous provider had shuttered its doors with little notice, prompting an emergency situation. However, since provider had also used ServTracker, they agreed to let the developers at CaseWorthy to migrate the consumer demographic data from their system to LifeCare Alliance’s. 

By migrating this client data electronically, LifeCare Alliance was saved 250 hours of manually entering over 4,000 records, allowing them to quickly begin serving the individuals in these counties with minimal service disruption. This migration additionally saved them close to $7,000 in temporary data entry staffing.

What's Next

Mobile Meals, Change of Condition Reporting, and Serving the Whole Person

Mobile Meals and mobile home care are next on the roadmap. Moving delivery records off paper will replace roughly 500 printed pages a day, eliminate seven years of off-site paper storage, and let auditors work directly from the system in real time.

The change-of-condition workflow inside mobile delivery will be just as important. Today, LifeCare Alliance relies on volunteer drivers remembering to report observations when a client seems off. A structured mobile workflow will let drivers flag those changes in the moment so a supervisor can follow up the same day, whether that means a phone call or a home visit. 

The team is also focused on serving the whole person. Because many clients receive multiple services, LifeCare Alliance sees an opportunity to connect information across meals, home care, and other programs so that a change noted in one service prompts appropriate outreach in the others. The capability is already in place inside ServTracker; the next step is putting it into daily practice.
