Element by Arkhe Consortium Health is a clinically integrated network (CIN) based in Charlotte, North Carolina, bringing affordable, high‑value technology to providers serving underserved and rural communities. As part of North Carolina’s tailored care management (TCM) program, Element acts as an administrative and technology backbone for small providers who could not independently afford or implement enterprise‑level platforms like MediSked, giving small and rural agencies access to enterprise-level care management, automated ADT alerts, and workflow-driven compliance tools they couldn’t implement independently.

Element supports 24 provider organizations, more than 35,000–40,000 covered lives, and over 100 care managers across participating agencies. By aggregating demand and centralizing implementation, training, and help desk support, Element enables agencies to participate fully in TCM and value‑based care programs.

My name is Jordan Toledo. I’m the senior director of operations. Element is a clinically integrated network. We provide affordable technology to providers who serve underserved populations.

We really needed to find something that was ready to go and already understood the regulations and compliance requirements for tailored care management. Member tracking with TCM, you have to track every single piece of engagement that you do, right? Every single phone call, every single other care team that you interact with, and also being able to report back on that for things such as value-based care.

One of the major things that we started when the platform was implemented was a value-based care program with Alliance. And a big thing we needed to be able to do was to be able to really extract that data that comes from Alliance, ingest it into MediSked to be able to report out on, you know, engagement, and to be able to allow them to have higher engagement numbers with less administrative burden.

With our data insights and the HEDIS measures for VBC, we’ve had a lot of providers that really doubled or tripled their engagement with closing the FUH gap. So that really is attributed to the ADT alerts coming in timely. Obviously, they’re not going to be perfect every single time, but them coming into the system, notifying the care manager immediately, and then also getting that email alert to know that it’s there helps them to really act on it quickly. And then at the same time, them being able to view their previous hospitalizations and able to kick off that workflow of what’s needed for the follow-up has helped them close gaps for, for value-based care.

The support that we get from MediSked and their willingness to kind of talk through challenges we have and find solutions for them is something that’s hard to find with a technology vendor, so we definitely appreciate that.
